POSITION SUMMARY The Resource Specialist supports the CHS mission by helping individuals with developmental disabilities not eligible for Medicaid and their families to identify and link them to resources.

Requirements

  • Excellent verbal and written communication skills.
  • Excellent interpersonal and customer service skills.
  • Excellent organizational skills and attention to detail.
  • Strong problem-solving skills.
  • Proficient with Microsoft Office Suite or related software.
  • Associate degree along with 1 year experience in a related field or an equivalent combination of experience and education may be considered.
  • Must have reliable transportation and an acceptable driving record. Must use your own vehicle for transportation.
  • Must be able to drive for work-related assignments to individuals’ homes to meet face-to-face with individuals on caseloads.
  • Must have a valid driver’s license. A class E Missouri Driver’s license if residing in the state of Missouri is preferred, but not required.
  • Must provide proof of personal, valid vehicle insurance.
  • Must have access to reliable internet for telecommuting employees.

Responsibilities

  • Manages a caseload of varying ages, disabilities, and case complexity of individuals by linking and connecting individuals and/or their families to resources and establishing strong, trusting relationships.
  • Creates a Milestone Plan by meeting at least annually with the individual or family and makes certain documentation is completed via written case note documentation for all activities for each individual within three business days of activity.
  • Maintains communication by taking phone calls and/or meets with families per their request.
  • Provides the most up-to-date information to individuals we support by participating in knowledge enrichment such as completing all training as assigned, continuing one’s education, and attending conferences and seminars.
  • Creates team unity by accepting and willingly carrying out special assignments or duties.
