About The Position

Position Summary: The Connecting NJ Resource Specialist works to refer and link women of childbearing age and caregivers of young children (0 to 5) to needed services in their region. The specialist receives and processes all risk assessment screens and refers to eligible social services and home visitation service. The Specialist will work with area MCH providers to provide linkages to other appropriate maternal and child health services in the respective County. The Specialist will support with Pete’s Place, CJFHC’s Baby Pantry and will process and order items requested by Community Partners. Requirements

  • Education: H.S. Diploma, BA Preferred
  • Experience: At least two (2) years of data entry experience excellent computer skills, minimum of 1 year experience with maternal child health, public speaking, communication and problem solving skills and a commitment to maternal/child health care issues.
  • Computer proficiency required with Microsoft programs including Windows, Outlook, Office 2007, Access, Excel, Power Point, Publisher and Word. Experience with Assuresign is preferred. Strong interpersonal & oral/written communication skills required.
  • Valid Driver’s license with the ability to provide proof of ongoing automobile insurance coverage is required. This position will entail reimbursable local and statewide travel utilizing personal vehicle.
  • Employees are required to occasionally lift up to twenty pounds of materials or equipment.

Responsibilities

  • Collects and processes all referrals received in the Connecting NJ HUB and through Pete’s Place: CJFHC’s Baby Pantry.
  • Works closely with Community Health worker (CHW) within their respective county and municipality to ensure coordination of services.
  • Conducts initial assessment via phone of client information to identify eligibility for home visitation programs and other medical and supportive services
  • Links and refers women of childbearing age, infants, and children to services as needed
  • Refers women and children, who have been identified as “lost to care”, to appropriate medical and social services
  • Ensures that screens are processed in a timely manner
  • Fields incoming calls to determine appropriate services
  • Prepares outreach packages for new partners
  • Participates on workgroups and attends community meetings as needed
  • Conducts outreach through phone calls, face to face meetings/ presentations, and mailings to providers to encourage referral submission as needed.
  • Reviews safe sleep protocol, Car seat safety strategies and hold harmless agreements with parents/givers who request items from the Baby Pantry to minimize risk to the baby and CJFHC.
  • Collects electronic signatures using Assuresign from parents/caregivers who order items through Pete’s Place.
