About The Position

Brookhaven Hospice is seeking a Full Time Compassionate Resource Nurse (RN) to provide patient care in and around the Bedford Area. Resource Nurses are responsible for effectively representing Hospice programs to patients, families, and others interested in hospice through pre-assessments, information sharing, and/or admissions in a patient's home or a healthcare facility. This role involves collaboration with the intake team, referring physicians, and other referral sources to promote the hospice philosophy of care. The schedule is Monday - Friday, 11 am - 7:30 pm, covering Bedford, Manchester, and Concord. Requirements

  • Hold current unencumbered license as a Registered Nurse.
  • Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
  • Must be computer proficient in typing and various programs, including background in EMR.
  • Possess and maintains current CPR certification.
  • Current driver's license, vehicle insurance, and access to dependable transportation.

Nice To Haves

  • Bachelor's degree in nursing or health care preferred.
  • Hospice/home health experience preferred but not required.

Responsibilities

  • Reviews clinical referral information and responds to request and inquires as appropriate.
  • Collaborates with Clinical Director regarding the eligibility and appropriateness of client for hospice services.
  • Provides information, pre-assessment, and admission services to patients who have been referred to the hospice; conducting pre-assessments visits in the home or healthcare facility as necessary.
  • Communicates with the Intake Department, the referring physician and the Hospice Medical Director to assure appropriateness of each referral and that the necessary certifications are obtained.
  • Applies nursing principles and techniques in the treatment of patients.
  • Provides nursing care, emotional support and education to patients and families.
  • Encourages patients and family members to participate in the patient's plan of care to meet established goals.
  • Participates in On-Call duties as assigned and defined by Clinical Director and Program Director.
