As a member of the health care team, the Resource Pool Registered Nurse (RN) will deliver high-quality, compassionate care focused on improving patient health and wellness. The Resource Pool RN will perform and coordinate patient care utilizing clinical judgment and nursing specific skills. This role will rotate between our primary care, express care and outreach sites and may support special clinical initiatives or projects. The Resource Pool RN will undergo training to gain and maintain competencies for all sites in order to confidently float to any CVCH location or department. Job Specific Competencies RN Leadership: Facilitates and provides direction to the health care team in delivery of quality patient care. Communicates effectively and respectfully with each team member. Coordinates flow and assists in providing patient care. Functions as a clinical resource and mentor for staff regarding patient care concerns and participates in training of health care staff. Collaborates with other health care agencies and services providing care coordination. Participates as a preceptor orienting new RN staff members to CVCH. Models standards of behavior and adheres to the Code of Conduct in all aspects of job performance. Patient Care Maintains RN Competency in all nursing skills essential to job function. Responsible for the RN assessment and prioritization of patient needs. Formulates a nursing plan based on assessed needs. Performs nursing tasks as appropriate within RN scope of practice. Documents and maintains appropriate records in the Electronic Medical Record (EMR). Triage Manages and directs incoming triage telephone calls. Completes triage assessment for scheduled and walk-in patients as needed. Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response. Collaborates with provider for appropriate plan of treatment. Follows policies, procedures, and protocols to ensure consistency and departmental effectiveness as well as improved health care outcomes of patients/callers and their access to appropriate health care. Assures that pertinent medical information is forwarded to proper departments as needed for continuity of patient care. Documents triage assessment, plan, implementation, evaluation, and follow-up in electronic health record (EHR). Care Coordination Identifies patients meeting criteria for case management (e.g., multiple chronic conditions, repeated health crises, high social risk) through registries, risk stratification, and provider referral. Develops a collaborative care plan based on provider treatment plan and patient/family self-management goals. Monitors patient/family response to plan of care and revises the care plan as indicated. Coordinates patient care through ongoing collaboration with provider, patient/family, community, and other members of the health care team. Facilitates the transition of care from inpatient settings, assisting with timely completion of post hospital follow up to including medication reconciliation, PCP/Specialist follow-up, education, and identifying problem solving barriers. Maintains required documentation for all care management activities in Electronic Health Record (EMR). General Duties and Responsibilities 1.
