About The Position

The Resource Nurse is responsible for managing medication refill requests, ensuring accurate and timely prescription processing, and coordinating with healthcare providers to maintain optimal patient care. The RN will assess patient needs, review medical records for compliance, and collaborate with pharmacists and physicians to optimize medication management. The Resource Nurse will be responsible for assessing patient conditions over the phone or through virtual communication, prioritizing care based on urgency, and providing guidance on appropriate next steps.

  • Utilizes the nursing process; assessment, planning, intervention, & evaluation to coordinate and deliver care.
  • Up to date knowledge of legal regulations and best practices in healthcare.
  • Must be able to communicate with different patient populations and multi-disciplinary teams.
  • Active registered nurse license from the state of Texas
  • Current BLS certification
  • Valid state Driver’s License
  • 1 year of related clinical nursing experience in an ambulatory or hospital setting.
  • Word Processing Skills - Prepares more complex documents in Microsoft Word including creating tables, charts, graphs, and other elements.
  • Spreadsheet Skills - Uses Microsoft Excel to analyze data, including the use of formulas, functions, lookup tables and other standard spreadsheet elements.
  • Graphics/Presentation Skills - Creates basic presentations in Microsoft PowerPoint.
  • Uses HRIS (Human Resource Information System) to manage HR (Human Resource) employee record keeping, workforce reporting, talent management, time & attendance, and benefits administration
  • Uses CRM or like software to manage and evaluate customer/patient information for business development of process improvement opportunities
  • Required Education: Associate in Nursing (ADN)

Nice To Haves

  • Preferred Education: Bachelor’s degree in Nursing (BSN)
  • 2 or more years’ of related clinical experience in an ambulatory/medical or hospital setting.

Responsibilities

  • Review and process medication refill requests in compliance with state regulations and clinical standing delegated order.
  • Assess patient medication history and adherence to prescribed treatments.
  • Communicate with providers to clarify prescriptions, dosage adjustments, and renewals as needed.
  • Educate patients on medication use, side effects, and adherence strategies.
  • Ensure accurate documentation of refill approvals, denials, and provider communications.
  • Collaborate with pharmacists, providers, and other healthcare professionals to improve patient outcomes.
  • Identify potential drug interactions or contraindications and escalate concerns accordingly.
  • Complete medication prior authorizations.
  • Conduct telephone or virtual assessments to determine the severity of symptoms and recommend appropriate care.
  • Prioritizing and routing patients to receive appropriate medical care or treatment
  • Utilizing established triage protocols to determine the appropriate level of care for each patient, whether it be immediate treatment, observation, or referral to another healthcare provider.
  • Managing and triaging after-hour patient calls to determine the next steps for patient care
  • Accurately documenting patient assessments, interventions, and treatment plans in the electronic medical record.
  • Maintain patient confidentiality and adhere to HIPAA regulations.
  • Stay up to date with current medical practices, guidelines, and regulations to provide the highest standard of care.
  • Participate in quality improvement initiatives related to medication management.
  • Effectively carries out tasks and responsibilities beyond core job duties and primary role. The additional duties may vary from time to time and encompass a wide range of activities that contribute to the overall success of the organization (floating, schedule variations, assisting co-workers, patients, visitors, customers, leaders, and other stakeholders in support of the organization.
