As a Resource Navigator at SpringPath Mental Health Services, you will play a crucial role in executing customized treatment plans that deliver real results and exceed patients' expectations. This position is designed for individuals who are motivated to enhance the quality of life for clients facing mental health challenges. You will be responsible for conducting crisis assessments and managing medical or behavioral emergencies, ensuring that clients receive the appropriate support and interventions during critical times. Your role will involve a variety of assessments, including suicidal assessments, vestibular assessments, and evaluations of immediate needs, functional capabilities, living skills, and resource acquisitions. In addition to assessments, you will be tasked with completing interpretative summaries and developing comprehensive crisis, treatment, and behavioral plans tailored to each client's unique needs. Writing progress notes, summaries, and discharge reports will be essential components of your responsibilities, as will providing both group and individual counseling services. You will also engage in skills teaching and case management, helping clients navigate their treatment and rehabilitation activities effectively. Your work will require you to implement treatment strategies, administer medications, and participate in weekly staff meetings to discuss significant incidents or accidents. Establishing high levels of mutual trust and support with clients and colleagues is vital, as is creating a coaching and learning environment that promotes consumer choice and independence. You will be expected to conduct yourself in a professional and ethical manner, handle information confidentially, and communicate openly and honestly with all stakeholders. Additionally, you will need to notify the proper authorities of any health hazards and observe established policies regarding health, safety, security, and company regulations. Other duties may be assigned as necessary to support the overall mission of the organization.
