Job Summary Serves as a subject matter expert in case management, offering operational expertise within the case management departments across multiple Scion Health hospitals Oversees the functions to coordinate, negotiate, procure services for, and manage the care of complex patients with all health team members throughout the facility Patient population served range in age from 18 years to geriatric experiencing medically complex conditions, general medical-surgical conditions with rehabilitation and/or wound care needs Integrates Case Management within the hospital’s primary functions, policies and procedures and standards of practice in a manner that contributes to the provision of comprehensive health care and achievement of quality cost effective outcomes
