About The Position

The Resource Coordinator supports individuals with intellectual and developmental disabilities and their families by handling all calls for information and referral regarding Community Access Unlimited (CAU) services and programs. Develop individual contacts with referral sources. Provide referral information to individuals if CAU programs are not viable. Responsible for implementation of program policies. Coordinate intake process from initial referral to admission into CAU services.

Requirements

  • Bachelor’s degree in human services, Social Work, Psychology, Special Education, or related field preferred.
  • Minimum of 1–2 years of experience working with individuals with intellectual and/or developmental disabilities
  • Strong written and verbal communication skills

Responsibilities

  • Serve as the initial point of contact for individuals and families seeking services
  • Complete intake assessments, gather required documentation, and verify DDD eligibility status
  • Assist individuals in understanding DDD service pathways, eligibility tiers, and available supports
  • Enhance the current level of contractual services within our departments
  • Maintain accurate and compliant records in accordance with DDD, Medicaid, and agency standards
  • Identify appropriate DDD-funded and community-based services based on individual needs and preferences
  • Collaborate with Support Coordinators to facilitate service initiation and continuity of care
  • Track referrals and follow up to ensure services are successfully accessed
  • Increase public relations and community education effectiveness.
  • Research and participate in various conferences, seminars, and community fairs and other events to showcase CAU services.
  • Assist in the creation and maintenance of marketing materials that showcase CAU services.
  • Assist in the Agency’s public relations efforts and community education as necessary
  • Report monthly on vacancies and admissions into CAU services
