About The Position

Under the direction and immediate supervision of the Director of Stabilization Home services, supervises and guides all individual community and home activities. Manages all staff assigned to the home, participates as a member of the Community Support Team in the development and implementation of assigned individuals' service plans, and represents the Agency in leadership and administrative activities. Ensures a positive, normalized home environment.

Requirements

  • Requires the knowledge, skill, training, and experience equivalent to a Bachelor's degree in one of the behavioral sciences.
  • Progressive responsible experience may be substituted for an earned degree.
  • An understanding of the causes of mental and developmental disabilities, and habilitation or intervention techniques used with persons who are disabled is required.
  • Possesses excellent human relations skills and is capable of planning, organizing, and carrying out community-based habilitation activities as well as developing and providing education and direction to assigned staff.
  • The physical activities for this position involve: Safety Care-based physical management, balancing, pulling, pushing, standing, climbing, grasping, stooping, crouching, crawling, kneeling, reaching, lifting, repetitive motion, pushing wheelchairs, and lifting up to 40 pounds.
  • Able to pass NADD DSP certification exam within 60 days of hire.

Responsibilities

  • Serves as a fully participating member of the home’s clinical interventions, including de-escalation and physical management.
  • Provides for the full and consistent implementation of each individual's service plan, instructs independent living coaches in the proper methods of conducting individualized programs and activities.
  • Monitors the appropriateness of rising and bedtime hours, bathing practices, meals and meal time settings, clothing and attire, and matters of individual and home safety, etc.
  • Encourages individuals to engage in community activities and events, individually and on a small group basis.
  • Responsible for ensuring a safe, clean, homelike odor and smoke-free environment; monitors the overall physical condition of the home and external environment; assists individuals in personalizing and decorating their home; involves individuals in housekeeping efforts, including dusting, vacuuming, dish washing, bed-making, etc.
  • Responsible for initiating repair and maintenance requests, avoiding institutional practices, conducting regular home and environmental surveys.
  • Advises the Director of Stabilization Home services of any individual related nursing, medical, or physical health problems, monitors the implementation of any special dietary requirements for assigned individuals.
  • Assigns work schedules and evaluates the performance of ILC; is responsible for accurately recording and submitting staff attendance reports; monitors visitation practices of individuals and staff; ensures the appropriate use of the home telephone. Personally, fills shift vacancies where required.
  • Conducts regular home staff and individual meetings, communicates Agency Policy and Procedures.
  • Provides minutes of such meetings to the Director of Stabilization Home services together with individuals, plans and monitors activities into the community, consults with the Director of Stabilization Home services in developing a plan for the community integration of each individual.
  • Works collaboratively with the staff of the Supported Employment and Developmental Training Programs in seeking to maximize individual growth and personal effectiveness.
  • Prepares and oversees the preparation of applicable reports and records in a timely fashion.
  • Ensures acceptable completeness, clarity, and spelling in the preparation of such documents.
  • Assumes responsibility for tracking and charting such issues as injuries, clothing inventories, specific program needs of the individual or issues within the home, i.e. seizures, work attendance, appointments, etc.
  • Communicates with parents, guardians, staff, visitors, community agencies, etc. in a professional and courteous manner.
  • Meets with parents, friends, or guardians of the individuals on a regular basis.
  • Functions as a member of the Trinity Leadership Team; is actively involved in Community Living Program and Agency-wide committees and task groups; works with other Trinity staff members in developing and suggesting events and system improvements, anticipates potential problem areas, and communicates any identified problem areas or concerns to the Director of Stabilization Home services.
  • May participate in the selection/employment of ILC staff; may provide Pre-service and on-the-job training experiences.
  • May participate in the development of the home budget and related financial tracking.
  • Recommends cost-saving techniques.
  • Responsible for budgeting and purchasing of groceries and household supplies.
  • Monitors the appropriate use of supplies and materials.
  • May provide or arrange for transportation of individuals to and from recreational, social, work or similar activities and medical or other appointments.
  • Seeks opportunities for self-improvement, reads relevant clinical and management materials, shares ideas and suggestions with Trinity colleagues.
  • Works with home staff in encouraging their personal growth and knowledge.
  • Consults with the Residential Associate Director and/or Residential Coordinator.
  • Prepares for inspection by surveyors and is knowledgeable about what is assessed.
  • Other duties as assigned.
