About The Position

WHO WE ARE : The Children's Home of Wyoming Conference partners with children, families and communities throughout New York State to inspire hope, develop skills and cultivate healthy relationships for positive futures. $65,000-70,000 annually $2,500 Sign on bonus Position Summary: The social worker is responsible for providing counseling to children and families within an 8 bed residential program. Social workers are to address the presenting needs and challenges of the children and families served, which are identified through the treatment planning process. Social workers plan discharges and arrange for appropriate transitions. The Social Worker will act as the facilitator for treatment team meetings/case conferences and provide clinical consultation to team members as appropriate.

Requirements

  • Master's Degree in Social Work, Psychology or related field required .
  • Limited Permit, LMSW, LCSW, LMHC, LMFT, Licensed Psychologist, or similar required
  • Driver's License and ability to maintain insurance throughout employment required

Responsibilities

  • Promote the growth of the parent-child relationship as indicated by that plan, as well as the agency's philosophy of family centered practice.
  • Provide appropriate comprehensive case work interventions including case planning.
  • Serves as a member of the program management team
  • Acts as a participating member of the program on call system.
  • Provide individual, family, group counseling, and crisis intervention.
  • Ensure that agency standards of practice as well as State, Federal, and Accreditation standards are fully met. Exceed those standards whenever possible.
  • Promote and guide the development of increased adaptive coping skills, socialization skills, parenting skills, independent living skills etc. through active involvement in treatment planning process and ongoing review.
  • Continually evaluate the strengths and needs of children and families to provide appropriate intervention planning.
  • Complete required reports and documentation in a timely manner meeting all standards of content and professional presentation.
  • Ensure timely, accurate recording of all components of clients records, including; progress notes, treatment planning, conferences, case specific materials, and Case Records.
  • Maintain good relationships through collaboration and advocacy on client's behalf with DSS, community agencies, schools, program staff and other service providers.
