About The Position

The Residential Services Case Manager at Hope Cooperative plays a crucial role in supporting individuals transitioning from homelessness to permanent supportive housing. This position involves providing crisis intervention, counseling, and resource coordination to enhance clients' quality of life and promote independence. The Case Manager works collaboratively with a multi-disciplinary team to develop service plans, advocate for clients, and facilitate access to necessary services, all while ensuring compliance with relevant regulations and maintaining client confidentiality.

Requirements

  • 3 years of experience in crisis intervention and addiction counseling.
  • Ability to work with individuals with mental health issues or physical health conditions.
  • Experience with substance abuse problems, preferably with individuals who are dually diagnosed (mental health & substance abuse).
  • Knowledge of 12-Step Programs and personal experience with substance abuse recovery is highly desirable.
  • Lived experience as being unhoused or a mental health consumer or a family member/loved one of a mental health consumer is desirable.
  • Mental health crisis intervention experience is highly desirable.
  • Extensive experience using HMIS (Homeless Management Information System) and community referral process.
  • Valid California Driver's License and current vehicle insurance.

  • Experience in management or supervisory roles.
  • Familiarity with government loan forgiveness programs.
  • Experience in providing clinical supervision towards licensure.

Responsibilities

  • Support individuals with active listening and empathy, conveying a message of hope and recovery.
  • Observe, assess, intervene, and refer in situations involving clients' mental health symptoms, physical health symptoms, and substance use issues.
  • Develop service/goal plans with clients and meet with them as needed to encourage, support, and advocate for progress and skill development.
  • Monitor and support the housing application process as needed.
  • Assist clients with transportation in personal vehicles when necessary.
  • Work with a multi-disciplinary team to provide integrated psychosocial rehabilitative services to clients in the community and their homes.
  • Attend meetings to ensure continuity of care for residents.
  • Provide positive public relations, community referrals, information, and support by linking clients to needed resources.
  • Advocate with and on behalf of clients and their identified significant support persons.
  • Accurately complete all necessary forms, reports, and paperwork required by the program on a timely basis.
  • Support the Residential Service Coordinator in facilitating the planning of social, cultural, educational, supportive, and recreational group activities when applicable.
  • Shop for food and household supplies to meet program or client needs.
  • Ensure the living space is clean and well-kept, providing coaching and assistance with cleaning and personal hygiene as needed.
  • Act as a designated Mandated Reporter for observed or suspected abuse and neglect of vulnerable populations.
  • Follow all policies and procedures in the agency HIPAA Manual to ensure the protection and confidentiality of all Protected Health Information (PHI).
  • Ensure up-to-date tenant files are maintained per agency standards.
