Position Summary: The Care Coordinator promotes engagement and retention in treatment and facilitates an effective discharge plan whereby the individual successfully connects to the next level of care. Hourly Rate Starting at $22.00.

Requirements

  • A Bachelor’s Degree in psychology, human services or a related field required.
  • Must possess a valid driver’s license and safe/clear driving record.

Nice To Haves

  • One (1) year of relevant experience preferred with sensitivity to work with drug dependent people within the scope of this rehabilitation program.

Responsibilities

  • Acts as the primary point of contact for residents accessing services at the treatment facility.
  • Conducts comprehensive assessments of patients’ needs and developing individualized service plans.
  • Coordinates referrals to internal and external resources, including treatment programs, support services, and community agencies.
  • Advocates for patients to ensure access to needed services and resources.
  • Monitors patient progress and adjusts service plans as needed.
  • Maintains up to date patient records and documentation.
  • Collaborates with clinical and medical staff and other service providers to ensure coordinated care.
  • Leads or co-facilitates support groups for patients in recovery, providing a safe space for sharing experiences, discussing challenges, and offering mutual support.
  • Ensures patients’ insurance is active and up to date. Meets with patients if insurance is going to lapse and completes applications as needed.
  • Collects fees and records payments; coordinates accounts receivable to include the billing and insurance authorization processes; electronically prepares medical records for new admissions and accompanying information; processes client refunds; participating in clinical staff meetings; DDAP processing for DMHAS client tracking requirements admissions and discharges; completes client fee contracts; updates lobby boards; process late fee letters on a timely basis
