Requirements

• Bachelor’s degree required; a minimum of 1 year relevant experience preferred.
• Experience working with adults with Serious Mental Illness in a residential or supported living setting preferred; experience providing skills building teaching in group format preferred.
• CPR/First-Aid Certification within 6 months of hire or according to program requirements.
• Valid driver’s license required.

Responsibilities

• Maintains physical environment and program standards to CHR, DMHAS, TJC, and DPH licensing standards.
• Establish rapport and working relationship, utilizing person centered practices.
• Effectively utilizes appropriate communication methods/tools to transfer information to other staff regarding the daily events.
• Teaches and assists all residents in areas of rehabilitation including nutrition and wellness, meal preparation, hygiene, laundry, banking and budgeting, basic home care skills, substance use education and recovery skills.
• Assists/Manages administrative functions and tasks at the direction of the supervisor.
• Prepares and facilitates educational and skill building groups and one to one intervention as directed by the treatment plan.
• Monitors medications of all residents in accordance with the Adult Residential Medication policy and procedure.
• Collaborates and coordinates care with providers and families as needed.
• Assists residents in addressing medical and psychiatric needs.
• Maintains documentation in compliance with agency and program standards.
• Participates in staff meetings, supervision, training, and other meetings as assigned.
• Additional duties as required.
