About The Position

Community Solutions, Inc. is seeking a dedicated and passionate Residential Re-Entry Housing Placement Coordinator & CBT Facilitator to join our team at our Day Reporting Center (DRC) located in Santa Barbara, CA. Our Day Reporting Centers in Santa Barbara and Santa Maria, CA provide a community-based alternative to incarceration for clients on parole. CSI uses a curricula-driven, evidence-based model designed to assess the needs of each client to reduce individual risk factors. This position is responsible for facilitating stable housing placement for participants re-entering the community while also delivering evidence-based Cognitive Behavioral Training (CBT) programming and life skills services. This position supports participants in achieving long-term housing stability, behavioral change, and successful reintegration into the community.

Requirements

  • Must possess an Associate’s degree from as accredited institution, and
  • A minimum of two (2) years of experience as a RRH Placement Coordinator or similar position; or Four (4) years of experience as a RRH Placement Coordinator or similar position.
  • Excellent oral, written and interpersonal skills desired.
  • Experience with Microsoft Office.

Responsibilities

  • Identify, organize, maintain, and disseminate housing information conducive to long-term recovery and stable housing (e.g., shared, supportive, subsidized, clean and sober, moderate-income housing).
  • Locate market-rate housing opportunities from a variety of sources.
  • Create individualized housing and placement plans based on participant goals and needs.
  • Provide referrals to community housing and related support services.
  • Follow up on housing applications and support participants throughout the housing search process.
  • Facilitate housing meetings with parti to review opportunities, assist with applications, and educate participants on good tenant practices.
  • Ensure participants adhere to all Re-Entry Housing (RRH) requirements.
  • Facilitate evidence-based programing and other life skills programming.
  • Conduct group and individual sessions utilizing motivational interviewing techniques.
  • Evaluate participant progress and document outcomes.
  • Work collaboratively with participants to develop and implement a Case Management Plan (CMP).
  • Serve as a role model by modeling pro-social behaviors to participants and staff.
  • Maintain a working knowledge of and adhere to agency programs, policies, procedures, and code of ethics.
  • Engage and collaborate with community agencies and service providers to support participant success.
  • Perform other duties as assigned by the supervisor.
