The Residential Program Coordinator at Developmental Services, Inc. (DSI) is responsible for integrating, coordinating, and monitoring the active treatment program and medical services for clients in the Supported Living division. This role involves representing clients to ensure they receive necessary services and interventions from quality providers, maintaining person-centered plans, and supervising Team Managers to ensure effective service delivery. The coordinator will also be involved in training, documentation, compliance, and direct care as needed.

  • Write training plans and case conference agreements for clients in the Supported Living program.
  • Develop, write, and implement training plans for clients regarding medical issues.
  • Attend and participate in quarterly and annual case conferences and other meetings related to clients.
  • Complete and maintain all required paperwork for clients.
  • Review monthly data documentation and write monthly progress reports for clients.
  • Ensure documentation of Supported Living services is completed and accurate.
  • Coordinate the delivery of approved services between selected service providers.
  • Coordinate and evaluate the instructional activities provided to clients.
  • Complete incident reports and needed follow-ups within 24 hours of incident notification.
  • Participate in monthly Case Record Review committee meetings.
  • Supervise Team Managers and provide client-specific trainings.
  • Audit and approve the time of Team Managers and assist with auditing DSP time.
  • Ensure services are provided in compliance with all funding sources and governing bodies.
  • Meet monthly with the Regional Director and Senior Director to discuss personnel issues.
  • Assist the Director of Community Services as needed.
  • Assist with the review of billing and time entry for accuracy.
  • Provide direct care as needed and participate in the on-call rotation.
  • Display patience in all situations involving clients.
  • Respond appropriately to client's behavior via verbal and/or physical intervention.
  • Be available to answer staff questions and schedule meetings with Residential staff.
  • Plan, schedule, and conduct divisional orientation with new Residential Staff.
  • Maintain confidentiality of all clients and staff.
  • Ensure employees follow all departmental and agency policies.
  • Review planned and scheduled hours to ensure client budgets are utilized correctly.
  • Maintain working knowledge of company-wide systems and lead trainings as needed.
  • Assist with special projects as needed.
  • Assist in setting up new client home files and ensure documentation is completed.
  • Assist with home visits and ensure staff maintain records documenting client health care.
  • Be flexible in work hours, including some evening and weekend hours.
  • Assist in planning and providing instruction in various skills based on programming needs.
  • Follow all agency and departmental policies and procedures.
  • Prioritize multiple job functions and coordinate responsibilities.
  • Bachelor's degree or equivalent experience in a human service field preferred.
  • At least one year of experience working with the DD population, preferably in a residential setting.
  • Must possess or be able to obtain CPR and first aid certification annually.
  • Must possess or be able to obtain NCI certification annually.
  • Must have excellent oral and written communication skills and maintain confidentiality.
  • Maintain a valid Indiana driver's license and insurance on personal vehicle.
