Residential Nurse Coordinator Role: To oversee and provide comprehensive nursing services for youth accessing services at the Transitional Therapeutic Home, coordinate health care services with outside health care providers, and provides health care education to staff. Promote therapeutic, evidence-based, and culturally responsive program services, using OHS and JCS Models of Care and Practices as outlined in the CWES Contract. Description of Duties and Responsibilities: To promote services and implement strategies and activities that are consistent with the DHS's Family-Centered Model of Practice, the DHS's Child Welfare Model of Practice, JCS's Model of Practice, and the OHS Guiding Principles with the goal of promoting each Child's Safety, Permanency, and Well-Being. Assess the client upon admission: collect and document the client's current health status and medical history. Oversees medical paperwork, prescriptions and documentation. Make contacts with previous health care providers and/or attempt to identify current providers. Set priorities for nursing action in a logical sequence according to the client's needs and formulate a plan of care. The plan includes physiological, psychosocial and environmental factors along with client/family/placement education and client discharge planning. The care plan will be revised as necessary to meet the evolving needs of the client. Document accurate and ongoing assessment of patient status including client care, nursing intervention, client response to care provided, client needs, problems, capabilities, limitations, and progress toward goals. Documentation will include evidence of appropriate client/family/placement/worker teaching, and the understanding of these instructions. Acts as liaison for all participating health services agencies and clinics. Communicates significant changes in resident's status to the Shelter Director, staff as appropriate and outside providers as necessary/appropriate. Documents interventions, nursing or other, and client response accurately, legibly, timely, and in compliance with policies. Completes/maintain all necessary forms, logs and behavior contracts. Administer medications to shelter clients. Coordinates and reviews lab work. Trains, mentors, and supervises the medication managers at the Shelter. Create and maintain medication schedules for nursing staff. Follow appropriate procedure for obtaining, returning or cleaning/disposing of equipment/supplies. Physically restrain clients when they become a threat to themselves, others or the Shelter milieu. Maintains high standards of confidentiality of all employee and resident records and information. Provide CPR, First Aid, Medication Manager or other health training/teaching to staff as necessary. Promotes resident empowerment and self-care activities and hygiene. Provide professional health-related consultation and assistance to clients and all who are involved to support the plan of care for a client. Make clinical decisions using appropriate nursing resources. Recognizes lawful/ethical limitations to his/her role in the nursing process. Adhere to Social Work Code of Ethics. Promotes a safe and calm environment for client care. Other duties as assigned.
