About The Position

The Residential Living Care Coordinator is tasked with providing clinical support to residents who reside in residential living. This position is also tasked with case management and care coordination. To be successful in this role, you’ll need to have knowledge of/ experience in the following: Proficiency in Technology and Systems Management: Proficiency in Microsoft Office Suite and Medical Records software applications Strong Organizational Skills: Managing and coordinating multiple resident care plans and clinical needs. Strong Communication Skills: Clear written and verbal communication with residents, their families, and other departments in the organization.

  • RN or LPN licensures

Responsibilities

  • Collaborate with the Residential Living Administrator regarding residents who need additional support (Home Care, Home Health, Personal Care, Skilled Nursing, etc.) and assist with implementation of these services
  • Assist Residential Living Administrator with follow up visits/calls with RL residents returning to their cottage/apartment from a higher level of care or acute hospital setting
  • Manage the process for Residential Living monthly charges for medical services rendered.
  • Collaborate with RL team members on the organization and maintenance of charts and health records
  • Complete designated portion of new resident group orientation for each new RL resident within two weeks of their move in
  • Track and record the biennial physicals, TB screenings, and vaccinations in Electronic Health Record and file the hard copy in the resident's record.
