About The Position

The Residential Counselor at Community Healthlink plays a vital role in providing quality care to clients by ensuring they adhere to their individualized treatment plans. This position involves supporting clients in various aspects of daily living, including hygiene, medication management, and symptom management, with the ultimate goal of preparing them for independent living. The counselor will also facilitate transportation to appointments and engage clients in social and recreational activities.

Requirements

  • High school diploma/GED/HiSet required for Residential Counselor I position.
  • Bachelor's degree in psychology or human services preferred for Residential Counselor II position.
  • Ability to pass a CORI background check.

Responsibilities

  • Serve as the primary residential counselor for a specified number of clients as directed by the supervisor.
  • Develop, implement, and document treatment needs assessments and treatment plans as part of continuous quality improvement activities.
  • Assist clients with activities of daily living and foster a sense of safety in the community, including personal hygiene, money management, and symptom management.
  • Ensure healthful menu planning, food shopping, and meal preparation in coordination with clients.
  • Monitor clients' medications, assist with appointments, and ensure safe storage of medications.
  • Lead skill development or recreational groups as assigned, conducting at least two sessions per week.
  • Attend team meetings and case conferences, collaborating with other providers for designated clients.
  • Prepare daily log entries, incident reports, and other required paperwork to meet program standards.
  • Work with residents to maintain the residence, including chores and light housekeeping duties.
  • Provide transportation to clients as needed, which may include using personal or program vehicles.
