About The Position

The Providence Center Residential Case Manager Brick Academy provide clinical case management services in order to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders. Working with consumers to achieve the highest level of independence possible.

Requirements

  • High School Diploma/GED required; Associate's or Bachelor's degree preferred.
  • If no higher education degree, must have 5 years of relevant behavioral health experience.
  • Experience in residential facilities is preferred.
  • Bilingual Spanish preferred.
  • Must have valid driver's license, registration, current inspection and reliable, insured automobile with adequate liability coverage.

Responsibilities

  • Initiate treatment and advocate for consumer within network of care.
  • Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities.
  • Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy.
  • Provide case management and support to consumers.
  • Make assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment.
  • Teach consumers adult daily living skills assisting them in all aspects of their life in the group home and supervised apartments.
  • Initiate treatment and advocate for consumer within network of care.
  • Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities.
  • Transport consumers in own vehicle or TPC van (if available) to planned activities and appointments.
  • Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy.
  • Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
  • Other duties as assigned.
