About The Position

Our Residential Program is searching for a full-time Case Manager/ QIDP to oversee the supports and services provided to residents who live in four small (2-6 residents each), agency-owned homes in Charleston. It is the perfect opportunity to make a positive difference in the daily lives of those who we serve. The Case Manager/QIDP will: Develop trust and rapport with residents, families and guardians Work with a team of professionals and the individual to create service plans that guide individual programming Advocate for/with the residents to assure that their needs and personal choices are being recognized and respected Supervise direct care personnel in the residential homes, providing guidance and support concerning the services provided to residents Assist the Director in maintaining compliance with state environmental standards of assigned homes Complete and review paperwork related to the residents' annual planning, monthly progress reviews, benefits, and other service needs. The agency will provide a cell phone and laptop for work-related use for these duties. Will be part of a routine on-call rotation to address concerns after business hours. Duties may include, but are not limited to resolving staffing issues, providing support or assistance, or responding to emergencies. An agency-owned is used when staff are scheduled to be on-call.

Requirements

  • Must be able to provide proof of a bachelor’s degree from an accredited institution in a field related to human services or education (please contact us if you are unsure of whether your degree would be a match)
  • A valid driver’s license
  • One year of experience working with individuals with developmental disabilities (experience can be a combination of professional, volunteer or school-related)
  • Must pass a criminal background check, as required by the Healthcare Worker Background Check Act (225 ILCS 46), and a DCFS Child Abuse and Neglect background check
  • Must reside in the Charleston/Mattoon area or within 20 miles of Charleston.
  • Successfully complete and maintain American Heart Association or American Red Cross CPR/First Aid certification
  • Pass the Short Test of Functional Health Literacy in Adults (S-TOFHLA)
  • Successfully complete medication administration training and annual recertifications to maintain medication administration status
