About The Position

The Resident Support Specialist at Moorings Park is responsible for assisting Independent Living residents in navigating personal, social, and environmental challenges as they transition through various stages of life. This role emphasizes providing support to ensure residents' well-being and adjustment to life changes, including health issues and emotional challenges.

Requirements

  • Bachelor of Social Work (BSW) required.
  • Current State of Florida Social Work License a plus.
  • Previous experience in social work, particularly in supporting aging adults, is advantageous.
  • Excellent oral and written communication skills in the English language required.

Nice To Haves

  • Experience in emergency response during natural disasters.

Responsibilities

  • Assist residents in adjusting to life changes such as illnesses, loss, loneliness, and mental/emotional health issues.
  • Collaborate with the Home Health Agency to coordinate services for residents being discharged from the hospital.
  • Ensure residents are linked to appropriate services.
  • Follow up with resident pull cord incidents and provide wrap-around services to support positive resident outcomes.
  • Collaborate with the Director of Ministry and local Hospice organizations on end-of-life and bereavement programs.
  • Coordinate with The Chateau for resident Independent Living discharges.
  • Support transitions to a higher level of care (ALF/MC, SNF).
  • Act as a liaison and advocate for Moorings Park residents and their immediate family members.
  • Track the progress of Independent Living residents and identify those who may benefit from additional services and attend and support Life Care Meetings.
  • Participate in general education sessions for residents to highlight the role and value of social work.
  • Assist in the assessment process for new residents and annual wellness assessments for established residents at Moorings Park, when needed.
  • Perform other duties as assigned by the Resident Support Director (LCSW).
