What You’ll Do As the Resident Support Services Manager, you will serve as a trusted advocate and resource for residents and their families by: Assessing residents’ psychosocial needs and communicating findings to the care team Developing and implementing person-centered interventions Maintaining accurate and timely clinical documentation Connecting residents and families with community resources and support services Serving as a liaison between residents, families, and external agencies Collaborating with interdisciplinary teams to support holistic care Participating in care planning, case conferences, and discharge planning Addressing behavioral concerns to ensure safety and reduce unnecessary medication use Supporting gradual dose reduction through coordinated medication reviews Consulting with licensed social work professionals on complex cases What You Bring Strong verbal, written, and organizational skills Knowledge of clinical social work practices, especially in geriatric care Ability to handle sensitive information with professionalism and confidentiality A collaborative mindset and genuine compassion for residents and families Ready to advocate, support, and lead with heart? Requirements

  • Must meet all state-specific background check requirements prior to working in the facility
  • Associates or Bachelor’s Degree in a psychology-related field.
  • Must have a valid driver’s license.
  • Six months experience in a long-term care environment preferred.
  • Strong verbal, written, and organizational skills
  • Knowledge of clinical social work practices, especially in geriatric care
  • Ability to handle sensitive information with professionalism and confidentiality
  • A collaborative mindset and genuine compassion for residents and families

Nice To Haves

  • Experience working with vulnerable or geriatric populations strongly preferred
