About The Position

Responsible for performing a variety of duties to ascertain the needs of and provide support to seniors and families residing in Nevada HAND, Inc., (NHI) communities in fulfillment of NHI Resident Services Department (RSD) objectives.  The Resident Services Coordinator will:   1. Assist in devising tools and protocols for collecting data about and obtaining input from residents; (2) perform assessments to determine resident needs; (3) execute services and provide resources to advance the personal and professional development goals of residents; and (4) collaborate with residents and other community stakeholders to advance residents’ wellbeing.

Requirements

  • Bachelor’s Degree in social work, psychology, or a related field and two or more years of work or volunteer experience advancing the wellbeing of low-income seniors, families, working-age adults, and/or youth; or a high school diploma or equivalent with three or more years of work or volunteer experience advancing the same.
  • Obtain a sheriff's work card license.
  • Ability to read, write, and speak English.
  • Proficient reading, report writing, and grammar skills.
  • Proficient typing and PC skills.
  • Effective interpersonal relations, communication, and customer service skills.
  • Ability to work independently and with others.
  • Ability to organize and prioritize tasks and to work closely with residents, resident groups, management, and service partners.
  • Flexibility to adjust work schedule to accommodate seniors and families as needed (i.e. work on the weekends or after normal business hours).
  • Possess general knowledge of local, state, and federal agencies and private social service agencies and programs that serve low-come seniors and families.
  • Stooping - frequently bending body downward and forward by bending the spine at the waist, requiring full use of the lower extremities and back muscles.
  • Kneeling - frequently bending legs at knees to come to rest on knee or knees.
  • Reaching - frequently extending hand(s) or arm(s) in any direction.
  • Handling - frequently seizing, holding, grasping, turning, or otherwise working with hand or hands.
  • Ability to transport objects up to 25 lbs.
  • Exerting up-to 30 lbs. of force occasionally, and/or up-to 25lbs. of force frequently.
  • Ability to lift objects up to 25 pounds from the ground level to 4 feet in height and move them from shelf to shelf in storage.
  • Walking - moving about on foot to accomplish tasks.
  • Stairs - Ability to walk up and down stairs.
  • Regularly required to use hand to finger, handle, or feel, reach with hands and arms and talk or hear using telephone, computer keyboard, mouse and calculator while seated or standing at a desk.
  • Able to sit and stand for extended periods.
  • Specific vision abilities required by this job include close vision, depth perception, and ability to adjust focus.

  • Experience in local, state, and federal agencies and private social service agencies is a plus.

Responsibilities

  • Perform resident reviews to determine residents’ needs and enter assessment outcomes into the preferred database system for tracking and documentation.
  • Perform timely data entry into preferred database system to capture the efforts (work) being done with residents, including follow-up, to track the outcomes of those efforts.
  • Pull monthly reports to review efforts and make adjustments as needed.
  • Assist in constructing surveys, conducting surveys, and engaging with residents in a variety of ways to ascertain their needs and interests.
  • Implement programs, services, and activities that support residents to fulfill their personal and professional development goals.
  • Support resident financial literacy, education attainment / lifelong learning, health / wellness, and community engagement.
  • Provide individual financial coaching, as needed,
  • Coordinate and facilitate group education workshops, as required.
  • Assist residents with the completion of appropriate applications, as needed.
  • Support and maintain collaborations with other community-based service organizations and resources that support RSD objectives.
  • Maintain petty cash to provide senior and family programming and activities to support NHI community(ies).
  • Prepare a monthly calendar to inform and encourage resident participation in ongoing and special activities.
  • Maintain a file for personal, emergency, social service, and health referral information for each participating resident.
  • Provide information to residents and make referrals as requested. Respond to inquiries in a professional manner and respond to resident requests timely.
  • Serve as a resource liaison with community partners that support the residents’ needs.
  • Ability to secure resources for residents including the distribution of food and other essential items.
  • Required to travel to different sites and home offices as needed.
  • Collaborate with Property Management team to achieve mutual goals for successful tenancies and effective property operations as represented in the Hand Property Management Resident Services Matrix of Roles and Responsibilities.
  • Participate in bi-monthly property team meetings and daily check-in meetings with Property Management.
  • Complete bi-weekly Collaboration Agenda with on-site PM team.
  • Perform resident wellness checks as needed.
  • Prepare and distribute flyers, calendars, notifications, and other goods to residents.
  • Ensure the security and confidentiality of all resident information, including files and documents.
  • Work with staff to identify and remediate any safety hazards on property.
  • Complete any property incident reports within 24 hours of incident.
  • Maintain a clean and clutter-free workspace and storage closets for the safety of residents, vendors, and staff.
  • Perform other duties as from time to time may be determined necessary for the well-being of the total operation.
  • Coordinate specific work tasks with other personnel within the department as well as with other departments in order to ensure the smooth and accurate flow of work and information.
  • Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or re-designed upon individual circumstances.
