About The Position

The Resident Care Coordinator plays a crucial role in our assisted living community, ensuring that each resident's unique needs are met with compassion and dedication. This position serves as a vital link between residents, care staff, and other facility personnel, fostering an environment where residents feel safe, purposeful, and engaged. The coordinator is responsible for developing, managing, and implementing care plans tailored to the individual needs of residents, in compliance with government regulations and company policies. This includes conducting thorough assessments, coordinating with medical providers, and ensuring that all care plans are effectively communicated and executed. In addition to managing care plans, the Resident Care Coordinator encourages socialization among residents, helping them to capitalize on their abilities and maintain a sense of community. The role requires excellent customer service skills, as the coordinator will serve as a liaison between residents, their families, physicians, and other healthcare providers. This position also involves monitoring changes in residents' conditions, documenting these changes, and ensuring that appropriate interventions are implemented and followed up on. The Resident Care Coordinator will participate in prospective resident move-in assessments and family care conferences, providing oversight of medication management systems to ensure that all medications are current, properly stored, and disposed of when no longer needed. This role is essential in creating a supportive and nurturing environment for our seniors, allowing them to thrive at every stage of their lives.
