Responsibilities

• Develop, implement, modify, and communicate all resident assessments and care plans.
• Serve as a liaison between residents, families, physicians, and other healthcare providers regarding healthcare issues.
• Participate in prospective resident move-in assessments.
• Evaluate, report, monitor, and document changes in resident conditions, ensuring interventions are implemented and completed.
• Participate in resident and family care conferences.
• Provide oversight of medication management systems, ensuring medications are current, properly stored, and disposed of when discontinued.
