About The Position

The Valencia at Draper is looking for a new Resident Care Coordinator. This is a full-time position. Typically Monday through Friday but must be available on weekends as needed. At least six months of resident care coordinator experience required! Position Summary: Assist Wellness Director and Executive Director with management of community staff and to ensure quality care of residents. Department: Personal Care Reports To: Administrator and RN FLSA Status: Non-Exempt Summary of Responsibilities to Ensure Success Include the following: · Work with the Wellness Director to continually conduct reviews of each residents' needs using assessments, observations, incident reports, care provided documentation, quality assurance meeting/reviews and communication with staff and outside agencies to and ensure assessments/care plans are up to date and are reflective of residents' needs; properly implement care plans utilizing care team, outside agencies, and family as needed. · Work with Wellness Director to hire, train, and schedule care staff to ensure excellence in care services are provided. Implement proven quality assurance measures to continually evaluate level of quality care being provided by staff; determine areas of needed improvement of services provided by care staff; retrain staff and/or implement new policy to attempt improvement of services provided by care staff; repeat. · Work with Executive Director to ensure accurate levels of care have been communicated/charged to families through proactive meetings/conversations; Work with Wellness Director to adjust level of care hours for schedule; Work with Wellness Director to determine best times for hours to be added or removed from schedule; review new hours and schedule with Executive Director; ensure that new schedule is implemented properly through communication with care team.

Requirements

  • At least six months of resident care coordinator experience required!
  • One year certificate from college or technical school; or three to six months related experience and/or training; or equivalent combination of education and experience.

Responsibilities

  • Ensure quality care is provided to the residents.
  • Develop care schedule per residents' determined needs.
  • Assist Wellness Director in developing quality assurance standards of care
  • Ensure quality assurance standards of care are met by staff.
  • Monitor the weekly medications cycle
  • Schedule Lab work and follow up with doctors, treatment and medications for residents
  • Schedule care of residents with home health, hospice for nursing, physical therapy and occupational therapy
  • Supervise, hire, and train C.N.A. staff.
  • Hire staff to appropriately care for residents.
  • Train staff to meet or exceed facility standards of care.
  • Supervise staff in a professional manner.
  • Schedule staff in a manner consistent with meeting standards of care and meeting the payroll budget.
  • Be on call, carry phone and respond to call-ins, find replacements, if possible or work the shift.
  • Assist with scheduling and executing in-service and new employee orientation meetings.
  • Manage staff files according to Department of Health and Labor Commission regulations.
  • Assist Wellness Director/Executive Director with corrective action meetings with employees.
  • Assist Administrator with Marketing efforts.
  • Field phone calls from inquiries.
  • Provide tours of facility if needed.
  • Assist Marketing Director in follow-up of inquiries.
  • Assist in the move-in process of new residents.
  • Other duties as assigned.
