About The Position

Resident Care Coordinator Location: Eagle Lake Village Status: Full-Time | Exempt (Salary) Wage: $70,304 Make a Difference Every Day Are you a compassionate leader with a heart for seniors and a passion for quality care? We are looking for a Resident Care Coordinator who is compassionate and deeply committed to supporting older adults with dignity and respect. In this role, you’ll oversee our wellness and resident care programs while working closely with the Administrator to ensure a safe, supportive, and person-centered environment for residents, families, and staff. Requirements

  • Ability to speak, read, write, and understand English
  • Working knowledge of Title 22 regulations
  • Strong communication, organization, and problem-solving skills
  • Ability to remain calm, confident, and reassuring in emergency or high-stress situations
  • A genuine interest in and respect for older adults

Nice To Haves

  • Experience in long-term care and dementia care preferred

Responsibilities

  • Leading and overseeing the wellness, medication, residential care, and dementia care programs
  • Supervising and supporting care staff, ensuring appropriate staffing coverage for all shifts
  • Monitoring and ensuring accurate medication management in accordance with physician orders and company policies
  • Evaluating residents for changes in condition and coordinating care with physicians and healthcare partners
  • Responding to resident and family questions or concerns with compassion, professionalism, and follow-through
  • Developing and maintaining service plans that reflect resident preferences and support mental and emotional well-being
  • Assisting with move-ins, transfers, and move-outs, including assessments and medical record completion
  • Ensuring compliance with Title 22 regulations, resident rights, safety protocols, and emergency procedures
  • Supporting staff through training, orientation, coaching, and performance feedback
  • Responding calmly and confidently to emergencies, incidents, and accidents, ensuring proper documentation and follow-up
  • Collaborating with community-based and healthcare partners to support coordinated care
  • Participating in in-services, staff meetings, and ongoing professional development
  • Upholding confidentiality, professionalism, and a culture of respect at all times
  • Supporting a person-centered approach that honors each resident’s individuality and life story
  • Performing other related duties as assigned by the Executive Director
