The Resident Care Coordinator ensures that the residents are safe, well cared for and comfortable. Supports and maintains the delivery of personal services for assisted living residents. Monitors policies and procedures, employee performance, and implements training and quality assurance of employee performance and resident care. Guides line staff, and oversees medication room systems. Complies with federal, state and local standards for facility operation Incorporates Generations' Mission, Vision, and Values into their daily work and interactions with others. SCOPE OF JOB TASKS: Provides training and back up personal care to residents as outlined in the resident service plan. Provides training and back up performance of med assist duties. Provides direct medication room supervision. Assists in set up of resident’s phones, call systems, and pagers Provides information necessary in problem solving for residents, staff and family members when needed. Participates in training of all new staff members who are working in assisted living. Promotes teamwork and continuity of care at all times. Provides positive reinforcement or redirection as necessary. Monitors change of condition, falls and all other emergency situations. Coordinates changes in service and ensures that adequate documentation of such items occurs prior to leaving shift by performing shift assignments for caregivers. Ensures that proper documentation of unusual situations, injuries, alert charting, communication records and resident care notes occurs according to facility policy. Contributes to preparation of shift assignments for caregivers. Ensures compliance of Assisted Living Facility with federal, state and local standards. Participates in quality assurance program and development. Participates in mandatory staff training. Reads service plans on a regular basis to ensure that care is being provided as agreed upon by resident. Prepares for service conferences by updating service plans and including all the pertinent data. Provides input to nurse and administrator when services change from that as outlined in the service plan on a continual basis. Assists personnel and residents in establishing a home-like atmosphere. Ensures that equipment is used and maintained as designed. Submits requests for repair and replacement when necessary. Works closely with administrator and nurse to ensure completion of tracking, monitoring, auditing and charting duties. Aids in planning and shaping policy and procedure to result in high quality service environment for residents. Assists with and performs move in processing, tours. and marketing. Other duties as assigned.
