About The Position

Resident Care Coordinators play a pivotal role in ensuring the highest quality of care for our residents. They support the Wellness Director by managing administrative tasks and maintaining effective communication across departments, with families, and medical professionals. NOTE: This role is from 7p-7a, rotating weekends.

Requirements

  • Minimum of two years’ experience in a similar role.
  • Proficiency in administering medications as per doctor’s orders.
  • Excellent interpersonal and communication skills.
  • Professional demeanor and attire.
  • Flexibility to work shifts, including weekends and holidays.
  • Ability to work independently and collaboratively.
  • Competence in providing personal care and daily living assistance to residents.
  • Certified Nursing Aide or Medication Technician certification.
  • At least 18 years of age.
  • Successful completion of a background check, drug screen, TB test, and physical examination.
  • Regularly stand, talk, bend, stretch, pull, push, and lift.
  • Assist residents with daily activities such as bathing, dressing, dining, and exercising.
  • Communicate effectively with residents, families, and co-workers.
  • Possess close vision, depth perception, and focus adjustment abilities.

Responsibilities

  • Coordinate onboarding, training, and orientation for new care team members.
  • Audit and ensure completion of Relias/MyALF training and compliance.
  • Provide feedback on care non-compliance to the Wellness Director.
  • Attend daily standup meetings and report resident issues.
  • Monitor changes in residents’ medical conditions and communicate with the Wellness Director or healthcare providers.
  • Review medication orders daily and attend weekly Service Plan meetings.
  • Ensure caregiving team’s input is reflected in Service Plans and Yardi notes.
  • Dispense and order medications as directed, where permitted.
  • Monitor supplies and coordinate resident appointments.
  • Conduct weekly audits of the Medication Administration Record.
  • Facilitate new resident move-ins, ensuring smooth transition and communication of needs to staff.
  • Perform Quality Assurance Audits and report findings to the Wellness Director.
