About The Position

Kahala Nui, considered Hawaii's premier Senior Living Community, is looking for Full Time RN Resident Care Coordinator to join our team! The Resident Care Coordinator (RCC) provides oversight of the assessment and care planning process in the nursing department. The RCC is responsible for ensuring that MDS (Minimum Date Set) is accurate, complete, follows the CMS MDS RAI guidelines (for Medicare and Medicaid) and is submitted to the appropriate agency or provider in a timely manner. The RCC will conduct thorough reviews of potential resident admissions, including medical history, insurance coverage, and care needs to ensure appropriate placement. In addition, the RCC participates in department or unit quality improvement activities or staff education, as needed. This position is fully on-site. Remote or hybrid work is not available. Schedule is generally Monday - Fridays 8:00am - 4:30pm. May be asked to work outside these hours depending on business needs.

Requirements

  • Valid state of Hawaii Registered Nurse license in good standing
  • Proof of annual tuberculosis clearance in accordance with state regulations.
  • Valid CPR and First Aid certification
  • Proficient in the use of computer programs, preferably MS Word, Excel, and Outlook

Nice To Haves

  • 1 year previous MDS experience preferred
  • 1 year long term care nursing experience preferred
  • Training and certification in infection control management preferred
  • (Equivalent experience or education may be substituted at the hiring manager’s discretion for educational or work experience requirements)
