Under New Management! Come be a part of the new Silver Birch of Midtown (formerly Bridgewater of Midtown)! Silver Birch of Midtown is seeking an outstanding Resident Care Coordinator (RCC) to join our team!

Requirements

• Licensed Certified Caregiver in Arizona
• Experience working with seniors in the medical field
• Ability to work independently and as part of a team
• Strong organizational skills
• Ability to manage multiple priorities
• Current CPR and First Aide certificates
• Current fingerprint card
• Current food handler's card
• Ability to work weekends, holidays, and a flexible schedule
• Ability to actively cover on-call assignments
• Familiarity with EMR software

Responsibilities

• First point of contact for families, visitors, physicians, and providers
• Maintains staff communication from shift-to-shift
• Manages medications, including cycle fill
• Completes monthly audits
• Manages new resident ALIS profiles
• Ensures incident reports are completed
• Ensures ADLs completed and documented
• Prepares resident service plans
• Manages staff schedule and covers open shifts, as needed
• Coordinates transportation for residents
• Trains new Med Techs
