Requirements

  • Knowledge of regulations as they apply to the assessment process.
  • Knowledge of Case-mix reimbursement, Medicare reimbursement, and other issues related to effective and efficient provision of service.
  • Ability to monitor and maintain records.
  • Education equivalent to graduation from an accredited school of nursing.
  • Minimum of two years of professional nursing experience, in an institutional setting.
  • Or any equivalent combination of acceptable training and experience which has provided the knowledge, skills, and abilities cited above.
  • Acceptable training, experience, or knowledge in the specific specialty area cited above.
  • Possession of a license as a Registered Nurse issued by the Commonwealth of Pennsylvania.

Responsibilities

  • Assures that MDS assessments are completed by assigned disciplines on a timely basis.
  • reviews residents' charts for appropriate documentation to support the MDS.
  • identifies resident changes in condition that meet requirements for initiation of a new MDS.
  • assists the interdisciplinary care planning team in developing an individualized care plan for residents meeting medical, nursing, and psychosocial needs identified by RAPS that ensures measurable objectives and timetables to meet those objectives.
  • includes residents and/or families in the care planning process.
  • assures that the care plans are reviewed and revised as required.
  • reviews charts for Medicare compliance.
  • Performs related work as required.
