Job Summary: Conducts and coordinates the development and completion of the resident assessment in accordance with the requirements of this state and the policies and goals of this facility.

Requirements

  • High School Education/GED or equivalent: Preferred
  • Five years' technical experience: Preferred
  • Licensed Vocational Nurse (LVN) licensure in the state of practice: Required
  • Certified as a Resident Assessment Coordinator or complete certification within 6 months of hire: Required
  • Basic Life Support (BLS OR HS-BLS OR RQIBLS) Health Care Provider certification from approved vendor per AH policy: Required
  • Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required

Nice To Haves

  • Associate’s/Technical Degree or equivalent combination of education/related experience: Preferred
  • Registered Nurse (RN) licensure in the state of practice: Preferred

Responsibilities

  • Develops a written plan of care for each resident that identifies social problems/needs of the resident and the goals to be accomplished for each problem/need identified.
  • Ensures that social services components of the MDS are completed and signed on a timely basis.
  • Monitors the facility’s QI, QM, and survey reports relative to social services and make recommendations to correct identified problem areas.
  • Encourages the resident/family to participate in the development and review of his/her plan of care.
  • Assists in the scheduling of care plans and assessments to be presented and discussed at each committee meeting.
  • Coordinates the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
  • Ensures that the care plan includes measurable objectives and timetables to meet the resident’s medical, nursing, and mental and psychosocial needs as identified in the resident’s assessment.
  • Conducts and coordinates the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of RAPs and Triggers.
  • Monitors the facility’s QI and QM reports to ensure that appropriate corrective action can be implemented when potential problems areas occur.
  • Ensures that all assessments are completed and transmitted in a timely manner.
  • Reports problem areas to the administrator.
  • Conducts or coordinates the interviewing of each resident for the resident’s assessment.
  • Evaluates each resident’s condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
  • Performs administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Administrator as required.
  • Reviews departmental complaints and grievances from personnel and make written reports to the Administrator of action(s) taken.
  • Follow facility’s established procedures.
  • Assist the Quality Assessment & Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies.
  • Involve the resident/family in planning social service programs when possible.
  • Performs other job-related duties as assigned.
