Resident Assessment Coordinator
About The Position
The Resident Assessment (MDS) Coordinator & Resident Care Manager (RN) will assist with the coordination of the resident assessment process, care planning updates, Minimum Data Set (MDS) assessment scheduling and completion. This role plays an essential part of accurately gathering data to document the condition of the resident as a foundation for proper care planning. Work Schedule: Full-Time ; Monday-Friday from 9:00am-5:00pm About Mirabella ASU: Mirabella is home to more than 370 residents who have access to a wide array of amazing amenities and care services. As part of the continuum of care, Mirabella has a 21 bed Skilled Unit and a 20 bed memory care unit located on the second floor – both serve both existing residents and the Tempe community at large. Our residents have access to highly personalized care with 21 Private Suites/Bathrooms and a newly constructed environment (Completed in August 2020)! Requirements
- Registered Nurse (RN) license from an accredited program, school or university.
- Strong Assessment & Care Planning skills.
- 2-3 years of previous MDS experience.
- Understand state, federal and local regulations in relation to long-term care and skilled nursing requirements.
