As the Resident Advocacy Director (RAD), you will divide your time between two Navion communities in West Columbia and Lexington, South Carolina—Colonial Gardens and The Legacy of Lexington—serving as a key support in resident care, assisting with clinical coordination, family communication, and Navion Cares implementation to ensure continuity and quality of services. You will spearhead enrollment in Medicare care coordination programs, develop and implement personalized care strategies, and ensure cohesive collaboration across interdisciplinary teams and external partners. Additionally, using your unique training and experience, you will provide vitally necessary behavioral health services to residents.
