Provides education in group and individual settings for patients Identifies the learning needs of patients and/or families and assesses readiness to learn. Evaluates the patient’s understanding of information taught, provides reinforcement and follow up as necessary. Documents teaching activities and each patient’s response completely and accurately. Assists to develop and refine lecture and teaching materials. Assists with community education programs and ADA advisory meetings/activities as needed. Assists with keeping teaching sites supplied. Participates in quality and performance improvement activities. Follow OSHA, Infection Control and HIPPA guidelines. Maintains professional credentials and CME credits to maintain Works closely with researchers and Program Coordinator to ensure high quality education and a safe environment for patients/families and staff Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
