About The Position

Concerto Renal Services is seeking a Social Worker to join its team. This rapidly expanding business focuses on reimagining dialysis care in communities, ensuring patients with end-stage renal disease receive optimal care through a convenient, centralized, and continuous model. The Social Worker provides psychosocial evaluation and counseling to patients residing in Skilled Nursing Facilities or rehabilitation centers regarding dialysis-related issues. This role involves collaborating with a multidisciplinary treatment team to integrate psychosocial needs and concerns into patient care planning. Documentation in dialysis charts must adhere to facility policies, Medicare, and Quality Assessment Performance Improvement (QAPI) requirements. The position also requires facilitating communication, as needed, between the dialysis unit and the Social Services Department of the Skilled Nursing Facility. The Social Worker functions as a member of the Interdisciplinary team and reports directly to the Manager of Social Services.

Requirements

  • Masters degree in social work.
  • Must be licensed as an LSW or LCSW.
  • Unencumbered drivers license.
  • Intermediate computer skills and proficiency with MS Word, Excel, Outlook as well as becoming proficient with Concerto specific applications within 60 days of employment.

Responsibilities

  • Conduct psycho-social assessments on each patient and determine social work treatment plan compatible with a total medical treatment plan per CMS guidelines (initial, 90-day, annual and as needed for unstable patients).
  • Track CIPA/POC for dialysis patients and coordinate POC meetings with RN, RD, MD and patient.
  • Complete KDQOL, PHQ9/2, and other tools as required
  • Complete Quarterly Reviews and provide quarterly documentation in every patients charts
  • Provide an education packet to new patients regarding their Rights and Responsibilities, Grievance Procedure, treatment options, and the importance of treatment, and complete documentation that this was completed.
  • Complete monthly tracking of patients needed for QAPI.
  • Maintain professional working relationships with other staff of Concerto as well as in the skilled nursing facility.
  • Participate in administrative and staff programs as required, including attending staff meetings and in-service meetings.
  • Compile and develop educational materials to be used as teaching aids in education of patients and staff as needed.
