Requirements

• Advanced Practice Nursing degree with relevant certification.

Responsibilities

• Perform routine and urgent assessments for patients.
• Triaging consults based on patient acuity.
• Lead clinician for the remote patient monitoring program.
• Order, perform, and interpret laboratory and radiology tests.
• Prescribe medications and treatments as needed.
• Coordinate care and maintain accurate patient records.
• Engage with interdisciplinary teams to address patient needs.
