Requirements

Associate degree in multidisciplinary nursing.
5 years of experience in nursing, preferably in case management or home care.
Current RN State License.

Responsibilities

Assess patient and caller needs through telephonic interactions for readmission management and service referrals.
Provide telehealth monitoring to evaluate patient conditions and follow up on care plans.
Facilitate external patient transfers and bed placements for assigned facilities.
Develop and implement individualized plans of care based on patient assessments.
Coordinate issue resolution for patients, ensuring timely responses to concerns.
Document all interactions accurately within established guidelines.
Crosstrain in various roles within the call center for backup assistance.
Build collaborative relationships with healthcare teams.
Participate in case discussions and team meetings to enhance care coordination.
Adhere to HIPAA regulations.
