Our Pharmacists are considered a ‘Clinical Pharmacist', meaning you will have the opportunity to perform comprehensive medication reviews and targeted interviews. Therefore, clinical knowledge of current industry standards is vital. The Clinical Pharmacist is responsible for adhering to company policies and procedures and expected to help build team capabilities through leadership skills and follow up.

Requirements

• Bachelor's degree in Pharmacy or Doctorate in Pharmacy (PharmD)
• Active state of Ohio Pharmacist license in good standing
• Completion of a post-graduate year 1 residency (PGY1) and/or Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists
• Bilingual in Spanish/English, a plus
• Strong experience with Medication Therapy Management and comprehensive medication reviews
• Prior interface with OutcomesMTM and Mirixa, a plus
• Ability to work flexible schedule
• Strong work ethic and ability to collaborate with multiple partners

Responsibilities

• Ensuring clinical accuracy and execution of patient prescriptions
• Verification of hard copy and digital Rx's
• Communication with healthcare professionals and making sure that the Dr. call line is answered
• Supervision and execution of incoming and outgoing prescription transfers
• Perform DUR to identify medication therapy management eligible patients and identify clinical intervention per patient with follow up
• Ensure all legal, privacy and compliance requirements are met
• Other duties as appropriate
