Reporting to the Chief Medical Officer, the Remote Night Float Advanced Practice Practitioner will provide after-hours telehealth care and access to comprehensive health care for the individual and family across all ages and genders within the context of Patient Centered Medical Home (PCMH) policies and procedures. The clinician will exercise individual initiative and independent action to ensure high quality and comprehensive outpatient medical care and after-hours coverage. The two central tenets of PCMH lie in team-based care and care coordination. The night float advanced practice practitioner is expected to function as an integral part of the care team, as a leader and an engaged team member.

Requirements

  • Current NP/PA Massachusetts License.
  • Board Certified in respective specialty as applicable.
  • A minimum of two years working as a licensed NP/PA in an ambulatory care setting; experience in a Community Health Center preferred.
  • Must be available to provide after-hours care on a regular basis.
  • Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting.
  • Excellent written and verbal communication skills – bi-lingual helpful
  • Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive etc.
  • Excellent judgment and ability to solve problems in a timely manner.
  • Ability to adhere to strict confidentiality standards.
  • Demonstrated ability to work effectively both as a team member and independently.
  • Demonstrated ability to work in a culturally diverse and inclusive environment.

Responsibilities

  • Works as an integral member of the care team, which includes careful attention to customer service, population management, appropriate delegation, and participation in the health center's quality initiatives.
  • Conducts a program of care within their discipline’s scope of practice and in collaboration with Primary Care teams and supervising physician/provider as warranted, with responsibility for examination, diagnosis and treatment of patients.
  • Obtains history and physical examinations of the patients in an ambulatory setting.
  • Utilizes laboratory and other diagnostic services as appropriate.
  • At the direction of the Chief Medical Officer and/or Senior Medical Director, works with the Float team to provide patient access during normal business hours and coverage for after-hours care to OCHS patients.
  • Be assigned other clinical support work for the primary care teams such as Epic “in basket” /desktop management for PCPs (especially those on ETO), at the direction of the Chief Medical Officer and/or Senior Medical Director.
  • Renders services to patients of OCHS in a competent, professional, and ethical manner; makes a diagnosis and administers treatments based on evidence-based medicine whenever possible but always within accepted standards of care.
  • The night float advanced practitioner will prescribe buprenorphine containing medications as needed for the safe care of patients living with opioid use disorder.
  • Seeks outside advice and consultation as necessary; refers to specialty care and for confirmation of diagnosis and treatment when appropriate.
  • Experience or willingness to practice in team-based care
  • Participates in health promotion and disease prevention activities including screening, immunization and health education events sponsored by OCHS.
  • Takes part in quality assurance and patient care assessment activities.
  • Adheres to the documentation standards set for OCHS staff.
  • Performs emergency treatment consistent with established policies and procedures accepted standards of care.
  • Participates in and complies with all policies, procedures and reimbursement plans related to MassHealth and other state and federal reimbursement programs or commercial payors with which OCHS may contract or affiliate.
  • Must comply with all OCHS policies and procedures; all bylaws, rules, regulations and policies of any hospital or facility of which you are granted privileges; and all applicable state and federal law.
  • Fulfills the expectations of the advanced practitioner position including other duties as needed.
  • Other related duties and projects as assigned.
