Requirements
- High School Diploma/GED required; Bachelor's Degree in Nursing preferred.
- Minimum of 7 years of experience in clinical, case management, or related fields.
- Current State of PA RN licensure or valid multi-state licensure required.
- Preferred certification in Case Management and experience with diverse populations.
- Strong analytical skills with proficiency in MS Excel and understanding of healthcare systems.

Responsibilities
- Maintain oversight over a specified panel of members by performing ongoing assessments and identifying clinical interventions.

- Document all activities in compliance with applicable regulatory and business process requirements.
- Stay updated with CMS, state, and local regulations pertaining to case management standards.
