About The Position

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our client's Outbound Call Center team. This position is focused on making outreach to patients who are scheduled for home visits and telehealth visits. The ideal candidate will play a vital role in ensuring smooth communication and preparation for upcoming appointments by reaching out to patients, assisting providers with chart preparation, and conducting assessments. This role is fully remote, offering flexibility while still requiring strong organizational skills, clinical knowledge, and attention to detail.

Requirements

  • Must be a Licensed Practical Nurse (LPN) in good standing.
  • Solid understanding of patient care, medical terminology, and healthcare processes.
  • Strong customer service orientation, with the ability to handle patient inquiries, concerns, and provide empathetic support.
  • Exceptional attention to detail and ability to accurately document patient information and manage records.
  • Strong verbal communication skills, with the ability to speak clearly and professionally on the phone.
  • Proficiency in MS Office (Word, Excel, Outlook).
  • Experience with EMR systems (eCW is preferred).

Responsibilities

  • Make outbound calls to patients scheduled for home visits and telehealth visits to confirm appointments, provide necessary information, and initiate chart preparation.
  • Ensure timely and accurate communication with patients regarding their upcoming visits, helping them feel prepared and supported.
  • Review patient files and assist with chart preparation to assist the provider during the patient's appointment, ensuring all necessary information is available for the healthcare provider.
  • Verify patient records, update demographic information, and ensure all required documents are collected ahead of appointments.
  • Conduct phone assessments with patients to gather relevant medical and personal information that may impact their care and the upcoming visit.
  • Evaluate patients' health status, ask pertinent questions, and document responses to inform care planning and providers' needs.
  • Enter and update patient information into the Electronic Medical Record (EMR) system (eCW preferred), ensuring accuracy and completeness of data.
  • Maintain confidentiality and adhere to HIPAA guidelines when handling patient information.
  • Collaborate closely with healthcare providers to relay important information obtained during patient outreach and assessments.
  • Deliver exceptional customer service during all patient interactions, answering questions, resolving concerns, and ensuring patient satisfaction.
  • Follow up with patients as necessary to ensure they understand appointment details, check on any pre-appointment requirements, and confirm attendance.
  • Follow all company procedures, policies, and healthcare regulations related to patient communication, data privacy, and documentation.
  • Manage a high volume of patient outreach calls efficiently and effectively while maintaining accuracy and attention to detail.
