At Remarkable Hospice, we prioritize the patient's interests above all else, which is what makes our role as Social Workers so fulfilling. Our approach involves taking the time to truly understand each patient's unique story, which is essential in determining their needs for comfort, safety, and health within their own home. As a Social Worker, you will be responsible for the continuous improvement of total quality care, specifically addressing the psychosocial needs of our patients and clients. Your services will be delivered in accordance with the patient's plan of care, adhering to accepted professional standards, applicable laws, regulations, and our internal policies and procedures. In this full-time position, you will assess the psychosocial status of patients, establish and monitor care plans, and deliver services as directed by the physician's plan of care. You will report on patient conditions to supervisors and collaborate with healthcare team members to address the significant social and emotional needs related to health issues. Your role will also include providing social work services such as individual counseling, community resource planning, and crisis intervention. You will develop care plans in consultation with physicians and other team members, ensuring that your assessments are informed by a solid understanding of psychological, biological, physical, and social sciences. Documentation is a critical part of your responsibilities; you will prepare and submit timely, legible, and relevant documentation of treatment and interventions, as well as reports on patient progress. You will assist other team members in understanding the social, ethical, and emotional factors affecting health problems, and you will coordinate care for all assigned patients to ensure their needs are met. Your role will also involve utilizing community resources to support patients and their families, advising caregivers to promote patient progress, and actively communicating with the multidisciplinary healthcare team to ensure coordinated care and effective discharge planning. Participation in on-call duties, quality improvement measures, and in-service training will also be part of your responsibilities.
