About The Position

Provide general nursing care to individuals in RTR Residential homes by assisting Residential Management to coordinate the medical services for individuals and to assist staff in the provision of all healthcare services. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: Provide clinical consultation regarding complex healthcare issues or newly diagnosed conditions that require an in depth healthcare management plan. Review Physician Orders, including treatments and medications with the Residential Manager and/or Program Director. Periodic review of Individual’s dietary restrictions and residential menus with Residential Managers Follow hospital admissions and be the liaison between RTR and DDS for all medical information. Assist RTR Management with discharge planning. Review DDS’ Health Screen Recommendations with the Residential Managers and/or Program Directors to provide all individuals with healthcare services in accordance to DDS regulations. Attend DDS monthly meetings and/or other required meetings concerning healthcare planning. Attend family meetings concerning individual’s healthcare/medical issues. Provide as requested communication concerning medical issues to all ISP team members. Obtain MAP Certification. Assist RTR Management in Medication monitoring. Provide staff training as necessary. Participate in on-the-job training and provide coaching to new co-workers as directed. Adhere to agency safety standards. Perform additional duties, or substitute for others as required. Adhere to all RTR policies and procedures. Develop and provide all services in accordance with RTR's philosophies including staff and Board beliefs. WORK AT ANY OF OUR LOCATIONS! Requirements

  • Registered Nurse Diploma and one to two years related experience and/or training in a community health setting.
  • Must hold valid Registered Nurse Certification.
  • Valid Driver’s License.
  • Basic knowledge of Microsoft Word and the internet.
  • Successful completion of all state and agency mandated trainings.
  • Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies.
  • Must be able to perform each essential duty satisfactorily.

  • Must be fluent in English and have the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or government regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to communicate effectively with individuals under supervision, employees of the organization, families, and members of the community.
