The Rehabilitation Specialist will provide assistance to clients in gaining skills that are based on their individualized goals. These skills include, but are not limited to, medication management, developing life skills (i.e. cooking, money management, cleaning, parenting, and socializing); pursuing recreational and leisure activities, housing, transportation, legal, medical and financial assistance, and securing appropriate entitlements. The Rehabilitation Specialist will participate in weekly team meetings to coordinate client care and assist in developing person centered care plans. The Rehabilitation Specialist is also required to meet organization stands in timely and accurate documentation, which focuses on the progress and treatment of clients. The Rehabilitation Specialist will work with clients both in the community and in the office, but is primarily a community based position. Driver’s license required.
