About The Position

Monday - Friday 7:30 am – 12:30 pm. The individual needs to understand the impact of behavioral health symptoms on presentation and importance of leveraging community and family supports. Please note these are 5 hours shifts that are 100% field based. Pay rates for Union positions are set in the Collective Bargaining Agreement with SEIU Local 521. Each position has a total of six steps, Step 1 (entry level) through Step 6 (15 years of employment with the organization). Union staff are promoted from Step 1 to Step 2 upon their one-year anniversary. The current Step 1 rate for this position is $35.49/hr, and the current Step 6 rate for this position is $39.19/hr.

Requirements

Master's degree in a mental health-related field (e.g., child development, child psychology, counseling and guidance, counseling psychology, early childhood education, human services, social psychology, social science, social welfare, social work, or sociology) and two years’ full-time experience in the mental health field; OR Bachelor's degree (not necessarily mental health-related) and four years’ full-time experience in the mental health field; OR Associate's degree (not necessarily mental health-related) and six years’ full-time experience in the mental.
First Aid Certification or successful completion of First Aid Certification within 30 days of hire for certain Agency locations; (E) Certification for other Agency locations. (D)
Certification in writing 5150 holds or obtain certification within six months of hire. (E)
Purchasing and kitchen, experience, including basic nutrition; menu development, cooking techniques, safe food storage and kitchen sanitation practices, and kitchen supply management. (E)

Special Requirements: Must be able to meet and receive a criminal records clearance, as required by Title XXII, other licensing regulations and Momentum practices. (E)
Proven proficiency in typing—minimum 40 net wpm; computer literacy, including Internet search and a basic proficiency level in Microsoft Office applications and Windows-based databases; (E) familiarity with UNIX-based databases. (D)
A valid California Driver License, with access to a reliable vehicle or for driving a company vehicle, current proof of auto liability insurance, and a clean DMV record, with eligibility for a Class B license. (E)
Physical requirements needed to perform the essential functions of this job, with or without accommodation: 1. Responsibilities

Provide mental health rehabilitation and case management services for a varying caseload, coupled with involvement of their families, which assist and support clients in all areas of daily living and facilitate integration into the community. Duties may include any or all of the following: 1. Develop curriculum and facilitate groups and one-on-ones through rehabilitation counseling processes that help develop and strengthen life, coping, and independent living skills; develop curriculums. 2. Provide group facilitation through field and periodic outings to promote community integration. 3. Help prepare and assist clients and their families with client social integration into community life, housing, school, employment, and other life-sustaining and independent living skills. 4. Assist in budgeting and financial planning; address health and safety issues and how to respond to common emergencies. 5. Interface directly with clients and provide counseling, particularly during times of conflict and crisis intervention. 1. Assess the independent living skills of clients and their stability; 2. Alert other professional staff of adverse reactions to medication or medication avoidance; 3. When assessed as highly unstable, such as being a danger to self or others, write up 5150 holds (involuntary detentions) to determine the extent of the threat; de-escalate that threat; 4. Update treatment plans per Agency guidelines; 5. Provide appropriate consultation and back-up to support other staff in responding to client crisis; 6. Help clients strengthen problem-solving, socialization, and coping skills; 7. Encourage clients’ resourcefulness, self-empowerment, and understanding their diagnosis and/or illness. 6. Provide service to clients and clients’ families in linkage with professional and community resources. 1. Coordinate care with other programs; refer clients to Agency and non-Agency therapists, if appropriate; 2. May communicate with clients’ probation agents, conservators, and case managers, and the courts on an on-going basis; 3. Liaise/advocate for clients with outside agencies; help and educate clients with researching outside, independent living and making the most of community resources; liaise with representatives of those resources, such as property managers, on behalf of clients; support the families of clients through linkage to appropriate resources; 4. Work with clients’ families and educate them on mental health issues, facilitating the assistance process toward successful transitions.
Coordinate activities and provide food and other related services to support the smooth operation of Agency kitchen and residential facilities and communications and maintain facilities to payor and other regulatory health and safety standards. 1. Organize, prepare, and serve meals, and maintain safe hygiene and sanitation practices, working collaboratively with clients to learn these skills. 1. Provide menu planning, preparation, set up, and service/delivery of meals and snacks; 2. Perform kitchen and dining room sanitation processes and activities; 3. Perform basic cleaning, such as washing dishes and cleaning the kitchen and dining room. 2. Coordinate and perform activities which maintain proper food supplies and storage.
Complete the clinical records and other documentation, processes, and procedures that enable clients to receive these services in accordance with agency, payor and regulatory requirements, utilizing paper or computer technology. 1. Develop and implement individual service and treatment plans; 1. Consult with Clinicians to get assessments and treatment plans approved and to discuss clinical issues; 2. Document progress on a periodic basis, as directed; 3. Update treatment plans per Agency guidelines; 4. Perform annual psycho-social rehabilitation assessments; 2. Perform administrative admissions and/or discharge paperwork and procedures. 3. Enter and maintain up-to-date clinical documentation for each client file, including case notes for each client per Agency standards. 4. Track due dates for all paperwork to ensure quality standards are met and accurate and timely billings can be issued. 5. Perform general office work, including electronic and telephonic communications and processing of timecards on a timely basis; type up payment authorizations.
Provide transportation in personal or agency vehicles, including, but not limited to, the transport of clients and or program supplies, for agency business.
Perform First Aid in emergency situations at certain locations.
Provide medical follow-up assistance to clients in obtaining a primary care physician, coordinating care with medical doctors, and assisting with medical issues, schedules, appointments, and transportation.
Facilitate client insurance benefits to expedite treatment and ensure timely payment to representative payees.
Provide appropriate consultation and back-up support to other staff in responding to client crises.
Attend and participate in staff, program, agency and community meetings and training; coordinate events, as needed.
Interface with staff, clients, and visitors, as conditions require and in an unobtrusive manner.
Ensure compliance with applicable health and safety or other regulatory requirements.
Ensure program facilities are clean and well kept.
