About The Position

We are seeking a dynamic and experienced Rehabilitation Manager (PT or OT) to lead our therapy team and oversee daily rehabilitation services. This role combines clinical expertise with operational leadership to ensure high-quality patient care, staff development, regulatory compliance, and program growth. The ideal candidate is a motivated clinician with strong leadership skills, a passion for patient outcomes, and the ability to foster a collaborative, supportive team environment.

Requirements

  • Licensed Physical Therapist (PT) or Occupational Therapist (OT)
  • Graduate of an accredited program
  • At least 3 years of clinical experience
  • Strong organizational and communication skills
  • Passion for working with older adults

Nice To Haves

  • Previous SNF experience
  • Prior leadership or supervisory experience
  • Knowledge of PDPM and skilled nursing regulations
  • Experience with electronic medical records

Responsibilities

  • Lead the Therapy Team
  • Provide day-to-day direction and support to therapy staff

  • Oversee documentation and regulatory compliance (Medicare/PDPM)

  • Prepare for surveys and audits

  • Look for ways to improve services and resident outcomes
  • Support census growth and community relationships
  • Promote a team culture that's positive, respectful, and collaborative
