We are seeking a Rehabilitation Aide to assist, as directed by therapists, to ensure that quality care is being provided to each resident. Responsibilities include assisting in scheduling, transporting, transferring, and lifting patients as required for rehabilitation services, documenting and communicating any scheduling conflicts with residents, distributing schedules and other needed information to the appropriate facility member as directed by the Director of Rehab and therapy staff, arranging with the kitchen staff for early meal trays as directed by the Director of Rehab and therapy staff, reviewing Net Health and PDPM information as directed by the Director of Rehab to ensure the accuracy of our service delivery model, assisting the physical therapist or physical therapist assistant with treatment (including, but not limited to, ambulation, ROM, hot/cold packs), assisting the occupational therapist and occupational therapist assistant with treatment (including, but not limited to, ADLs and feeding programs), assisting the Speech-Language Pathologist with treatment (including, but not limited to, feeding groups, functional maintenance, and memory books), adjusting equipment as needed, maintaining general upkeep of therapy areas, performing clerical duties as assigned, remaining updated on the residents' medical and physical statuses, and attending in-services and staff meetings as directed.
