About The Position

The Rehab Social Work Case Manager utilizes professional clinical social work skills to assess patients' need and support systems for efficient discharge planning, psychosocial, counseling, and social drivers of health support needs. This role involves monitoring and updating interdisciplinary care plans as appropriate and establishing plans for effective management of the identified needs, both episodic and population-based. Assessment, planning, intervention, and reassessment are performed in a collaborative environment, where the case manager works closely with Medical Staff, Clinical Case Managers, Nursing, Allied Health disciplines, hospital business/financial services, and other stakeholders.

Requirements

• Master's degree in social work (MSW) from a CSWE accredited university.
• Minimum of three (3) years of acute care hospital or related healthcare experience; OR a minimum of two (2) years of acute care hospital case management experience.

Nice To Haves

• Active Case Management Certification (CCM, ACM-SW, A-SWCM) or within one (1) year of hire.
• Demonstrated ability to manage high risk situations and adjust priorities according to patient and department needs.
• Strong verbal and written communication skills, especially when on-call.
• Strong interpersonal skills and ability to work collaboratively with various stakeholders.
• Solid clinical social work assessment, critical thinking, problem-solving, interviewing, decision-making, negotiation, conflict management, and organizational skills.

Responsibilities

• Utilize clinical social work skills to assess patients' needs and support systems.
• Support efficient discharge planning and address psychosocial and counseling needs.
• Monitor and update interdisciplinary care plans as appropriate.
• Establish plans for effective management of identified needs.
• Collaborate with Medical Staff, Clinical Case Managers, Nursing, and Allied Health disciplines.
